Most general dentists treat specialist referrals like a one-way door: send the implant case out, hope the patient comes back, never think about it again. Meanwhile the practices that actually grow through referrals understand something different — a referral network is a two-way street, and the traffic only flows if you build the road on purpose.
Because nobody owns them. You refer to an oral surgeon or periodontist once, the case goes fine, and then the relationship runs on autopilot until it fades. There’s no system, no communication loop, no reason for that specialist to think of you when their patients need a general dentist. A relationship that isn’t tended is a relationship that’s shrinking — you’re just not measuring it, so you don’t notice until your new-patient numbers dip.
Reciprocity that’s real, not transactional. The GPs who win the referral game do three things consistently:
This is the strategic fork. As we talk about often on the podcast, the ideal model is usually a hybrid: bring in-house the high-volume, high-margin procedures you can do well and predictably, and build tight referral relationships for the rest. The math matters — every case you refer out is production leaving your practice, but every procedure you add before you’re ready is a quality and profitability risk. Know your numbers before you decide which door a case walks through.
Stop treating it as personal favors and start treating it as a channel. Track where your new patients come from. Know which specialists send you patients and which only take. Build the two or three relationships that actually reciprocate, and invest in them like the growth asset they are. A referral network built on purpose is one of the cheapest, most durable patient-acquisition channels in dentistry — and one almost nobody systematizes.
Referrals aren’t a courtesy — they’re a channel. Communicate like a partner, make the specialist’s life easy, decide deliberately what you keep in-house versus send out, and measure the flow in both directions. Build the road on purpose, and it carries patients both ways for years.
